Aloha Families — The Centers for Disease Control and the Food and Drug Administration have granted emergency use authorization for the Pfizer Covid Vaccine to be used for 5-11 year-olds beginning immediately. Kualapu`u School, in cooperation with Molokai General Hospital, would like to know if you are interested in having your child/children vaccinated, at what location you would like them vaccinated and if you want to be present when they are vaccinated.
It is our goal to make it as easy as possible for your child to receive the vaccine. Please take a few minutes to respond to the attached survey. Your answers will help Molokai General and our school with planning and implementation. Please complete this survey by Wednesday, November 10th.
