Also, if you or someone in your family has tested positive for Covid or is consider a close contact, please contact the DOH Contact Tracer, Shalia Jensen, at 808-856-9143.  She can give you guidance about quarantine/isolation requirements.  

For school purposes, a note from the DOH or your doctor is helpful for documenting either if your child is absent as a covid related absence such as close contact/quarantine/isolation, or a regular absence.  

If you are an employee of Kualapuu School, we will also require a note for your leave if it is related to Covid.
