Dear Parents of 6, 7, and 8 year old Children (1st, 2nd, 3rd graders):
Your child is invited to participate in Children’s Healthy Living Center study that will focus on improving children’s health in your community. We need your permission for your child to be part of this research program. CHL staff from the University of Hawaiʻi would like to meet with you to explain the program.
If you agree to your child’s participation, you will be asked to fill out several forms. Also, you will receive instructions on how to complete at home two food logs for your child. You will receive a $10 gift card as compensation for your time and effort.
At the same meeting or on another day, CHL staff will be measuring each participating child’s height, weight, and waist.
You will be asked to record what your child eats for two days. A few parents will be asked to return the food logs. Once the food logs are returned you will receive another $10 gift card as compensation for your efforts.
What is CHL Center? CHL Center is measuring young children in several communities in Hawaii, to follow-up on test methods of the CHL program, conducted from 2013 to 2015, that aimed to improve children’s health.
How will the participating children benefit? The children will know that they are helping with an important project.
If you would like more information about the Children’s Healthy Living (CHL) Center and to enroll your child in this important children’s health research program, an CHL will be available to meet with you on:
